How to use Anesto, step by step.

Getting started
Create your account
Open Anesto and use Sign in with Apple, or tap Use email instead and create an account with an email and password. Always sign in the same way: your cases belong to the account you used.
Tell Anesto who you are
- What should we call you? The name you enter goes on your PDF reports and your profile.
- Tell us about you: choose Attending Physician, Resident, CRNA or Fellow.
- Residents then pick their program from ACGME’s list, their current year (CA-0 to CA-3) and their primary hospital. Anesto works out your graduation date and tracks your pace from it.
- Fellows pick Adult Cardiothoracic, their program and their start and graduation dates.
- CRNAs answer How do you practice? (Independent, Medically Directed or Medically Supervised).
- Where do you mainly work? Name your main hospital or surgery center. You can add more sites any time.
Logging a case
By voice or by typing
On the Log tab, tap the round mic and say the case the way you’d tell a colleague: the procedure, then the airway, lines, blocks, ASA and age. Tap anywhere to stop. Or type the same thing and tap Log Case. While it works, the bar says “deciphering…”.
Never say or type patient names or identifiers. The line under the bar is there to remind you.
Shorthand works
“Lap chole”, “LMA”, “DLT”, “nasal RAE”: common shorthand is filed under one standard procedure name, and your own words stay on the case. Tap How to dictate next to the bar for the full guide.
Solid is recorded. Dashed is a guess.
A solid chip is on the record. A dashed chip is an educated guess, something likely that you didn’t say. Tap it to confirm and it turns solid. Nothing dashed ever prints.
A whole list at once
Say “4 lap choles” and you get four cases, ready to open and fill in. An ASA or age you say once goes on every case in the batch; the rest you edit per case.
Logging a case from another day
Under the bar, choose Today, Yesterday or Other day. Other day shows the past week, and Earlier than that… opens a calendar. While you’re on another day, an amber band says so, with Back to today. The day you chose stays for the next few cases, then goes back to today on its own.
Editing a case
Tap any case to open it, change what you need, and tap Save. If something required is missing, the button says what. When Anesto gets one wrong, this is the fix: open the case, tap, fix, done.
Deleting and getting a case back
- To delete: swipe left on a case and tap Delete, or open it and tap Delete this case at the bottom.
- Undo: a “Case deleted” message appears for a few seconds with Undo.
- Recently Deleted: on the Account tab. Cases you deleted in the last 30 days are there; tap Restore and the case comes back as it was.
- On ACGME: deleting in Anesto never deletes anything on ACGME, and restoring never sends a case again.
Hospital sites
Add sites on the Account tab under Hospital Sites → Add Hospital: a name, an optional short name, and what kind of place it is (hospital, surgery center or office). The star marks your default. You can also add one from the site pill on the Log tab. The site you pick sticks for the rest of the day.
Your supervising attending
Resident and fellow cases need a Supervising attending. Type a name or pick one you’ve used before. Once you’ve connected ACGME, pick from your ACGME attendings: a case with a typed name that isn’t on your ACGME list saves, but can’t be sent until you pick a name from the list.
Sending cases to ACGME
Connect once
On the Log tab, tap Connect your ACGME account and sign in on ACGME’s own page inside the app. Anesto never sees your password.
Send
In History, tap Send to ACGME on a case, or send all when several are ready. Anesto fills in ACGME’s form from what you logged, submits it, then checks your ACGME list. When it finds the case there, the case shows On ACGME with a green check.
When a case needs something first
If a case can’t be sent yet, it says what’s missing, like an ACGME attending, an ACGME site or an age band. Fill it in and send.
“Not verified”
ACGME accepted the submit but Anesto couldn’t find the case on your list afterward. Tap Re-check: it looks the case up on ACGME and never adds anything. If it still isn’t there, you can link it to an entry, send it again, or leave it for now. Check acgme.org before you send a case again.
“Removed on ACGME”
The case is no longer on your ACGME list. Choose Send again, Link it (to an entry you typed on acgme.org) or Leave it.
Changing a case that’s on ACGME
Anesto only adds cases. Changes you make in Anesto don’t reach ACGME; edit the case on acgme.org. More about how sending works →
Progress and your ACGME totals
The Progress tab shows every ACGME minimum. Each one you haven’t met shows how many more you need, the monthly pace to finish by graduation, and a short tip on where to find those cases. Badges say On Track, Watch or Behind.
Cases you logged before Anesto aren’t in these numbers until you tap Get my ACGME totals. Anesto reads your totals from ACGME and shows them beside its own, with the date it last read them. Want to check without the app? Try the ACGME pace calculator with your ACGME report.
Fellows: the TEE log and the fellowship report
- Your role on each case: Primary, Supervising or Not involved. It starts at Primary.
- Add a TEE: tap + TEE on a cardiac case, or + on the TEE log. Pick the date, the attending (required for reviews too) and Performed or Reviewed. You can save several exams at once.
- The two counters on the TEE log are the NBE’s: Performed toward 150, Total toward 300. An exam that isn’t perioperative is marked “not counted”.
- ICU month and conferences: check them off on the Account tab.
- Fellowship report: tap Export on the TEE log. It includes your summary, case log, TEE log, TEEs by month and a draft of the NBE letter for your program director.
Exporting your cases
- CSV, for everyone: tap Export CSV at the top of Progress or Stats. The file opens in the share sheet, so you can save it to Files, mail it or AirDrop it.
- Credentialing PDF, Pro for attendings and CRNAs: Account tab → Export PDF Report. It’s counted by facility, with practice capacity, ASA, age, specialty, technique and procedure.
- Custom date range: choose From and To, then Apply Filter. Pro for attendings and CRNAs; free for residents and fellows.
Backup and a new phone
Backup is automatic: your cases, hospital sites and a fellow’s TEE exams. It stays quiet when all is well. To check, look at Cloud backup on the Account tab, which says something like “All 101 cases backed up · 2 min ago”, with Back up now if you want it.
New phone? Install Anesto and sign in the same way as before. Your cases come back, and Anesto says “Welcome back” with how many were restored.
AI parsing and your case text
AI parsing is off until you agree to it. On the Account tab, under AI parsing:
- AI Parsing turns it on or off. Turning it on shows exactly what’s sent: case descriptions to Anthropic, voice to Groq. Turned off, you fill in case details yourself.
- Case text decides whether what you typed or said is kept in your account (“Kept in your account to improve Anesto”) or stays on this phone only.

Switching provider type
Account tab → Provider type, then pick Attending, CRNA, Resident or Fellow and confirm. Switching never wipes your details: each type keeps its own, and every case keeps the mode it was logged under. Switching to CRNA asks how you practice; switching to Fellow asks for your fellowship details if they’re missing.
Light or dark
Account tab → Theme: System follows your iPhone, or pick Light or Dark.
Pro: subscribing, restoring, cancelling
- Subscribe: from any Pro feature, choose monthly or annual and tap Try Pro Free for 7 Days (or Start Pro if no trial is offered).
- New phone or reinstall: tap Restore purchase on the same screen.
- Cancel: in Settings › Apple Account › Subscriptions on your iPhone, at least 24 hours before the period ends.

Getting in touch, and deleting your account
- Message Leo: the card at the top of the Account tab opens a chat with Anesto’s founder.
- Email: support@anesto.app.
- Delete your account: Account tab → About Anesto → Delete Account, then type DELETE to confirm. Your account and all its cloud data are deleted. Cases still on the phone stay until you delete the app.
